A.) The Wound Treatment Process
 
The treatment of non-healing wounds involves four main steps including : Assessment, Intervention, Follow-ups with re-evaluation, and Patient Education.

B.) Types of Wounds Treated
 
IWCC has facilities of treating all types of Wounds including :
 
 

Non healing Diabetic foot Ulcers ( DFUs ) :

 
  • No matter how old they are ( Months or Years old ).
  • No matter how big they are
  • No matter how many they are
  • Neuropathic Ulcers – They are because of loss of sensation in the feet. Diabetes is the usual cause.

  • Gangrene or Wounds with Pre Gangrene

  • Bone infections( Osteomyelitis )

  • Bed Sores or Pressure Ulcers - This type of ulcer is caused by excessive prolonged pressure on one area of the body. The most common place to see this type of ulcer is in a person confined to bed and they occur on at back, sacral area, heels &Shoulder.

    • Venous Ulcers ( Varicose Veins ) - These types of ulcers are due to compromised veins. They are commonly seen in the ‘Garter area' ( around the ) ankle and are slow to heal.
    • Arterial Ulcers of Peripheral Vascular Disease - Due to poor blood flow to the lower extremity. This type of ulcer can be very painful and is usually found on the margins of foot e.g. tips of toes, heels etc. They can be easily infected.

C.) Therapeutic Services
 
Our unit offers a full range of most advanced therapies for the treatment of wounds, including :
 
1.) VAC / NPWT Dressing :

Vacuum Assisted Closure(VAC) also known as Negative Pressure Wound Therapy(NPWT) is one of the most advanced and effective modality in wound care & has revolutionized the care of non-healing ulcers of all variety.

Since its introduction V.A.C .therapy has changed the way wounds are healed. With more published clinical evidence than any other form of Negative Pressure Wound Therapy, V.A.C.therapy has been selected as the treatment of choice for more than 8,000,000 wounds worldwide.

V.A.C. therapy promotes wound healing through Negative Pressure Wound Therapy (NPWT). By delivering negative pressure (a vacuum) at the wound site VAC dressing helps draw wound edges together, remove infectious materials and actively promote granulation tissue leading to early closure of wound.

     
2.) HBOT ( Hyper Baric Oxygen Therapy ) :

Hyperbaric oxygen therapy involves breathing 100% oxygen in a pressurized Chamber, which can accommodate one person(Monoplace Chamber) or number of persons(Multiplace Chamber). Hyperbaric oxygen therapy is a well-established treatment for decompression sickness, a hazard of scuba diving. Other conditions treated with hyperbaric oxygen therapy include serious infections, wounds that won't heal as a result of diabetes, gangrene or radiation injury, infection of bone(Osteo Myelitis) or skin and Skin graft or skin flap at risk of tissue death etc.

In a hyperbaric oxygen therapy chamber, the air pressure is increased to three times higher than normal air pressure. Under these conditions, lungs can gather more oxygen than would be possible breathing pure oxygen at normal air pressure. Dissolved portion of Oxygen in blood increases even up to 20 times than normal.Blood carries this oxygen throughout body. This helps fight bacteria and stimulate the release of substances called growth factors and stem cells, which promote healing.

3.) Ultrasonic Debridement :

Debridement means removal of dead necrotic & devitalized tissue from ulcer. With this modality there is Ultrasonic formation & collapse of Vapor Bubbleswho fragments & emulsifythe necrotic tissue without disturbing the viabletissue.

4.) Maggot's Therapy :

Special medicinal Maggot's of Lucilia sericata (Green Bottle fly) who eat only dead necrotic part of wound (and do not disturb the surrounding healthy skin) is used for the healing of chronic non-healing ulcers. These maggots debride wounds by dissolving the dead (necrotic), infected tissue, disinfect the wound by killing bacteria and stimulate wound healing.

These Maggots are very effective way of debriding problematic wounds but they are costly, difficult to get(only 2-3 companies worldwide manufacture them)& they die within 24 hours. Our director Dr.Ashok Damir has used these Maggots for wound care during his wound fellowship in USA but because of unavailability of them in India, we are unable to provide this therapy at our center.

5.) Ozone Therapy :
 

Ozone (O3) is a triatomic allotrope of Oxygen which is very strong oxidant and is > 3000 times powerful disinfectant than chlorine. It disinfects, oxidizes and deodorizes the wound.

Technique of giving Ozone therapy is known as “Bagging” 

which means method of isolating a body part by surrounding it with a plastic bag and introducing ozone.

 
6.) PRP Gel :
 

PRP also known as Platelet Rich Plasma is derived from patient's own venous blood. For making this gel, patient's blood is first of all centrifuged, then “Buffy Coat” is mixed with special reagents ( Vita. C, Thrombin, CaCl2 ) in specially designed double syringe . Reagents activate Platelets & Plasma and make gel consistency. This gel is applied over wound, which is found to be very effective in healing of long lasting wounds.

 
7.) Growth Factors :
 

They are naturally occurring protein capable of stimulating cellular proliferation and cellular differentiation.US FDA has approved only PDGF(Platelet Derived Growth Factor) and EGF(Epidermal Growth Factor) for wound care.

They are Chemo tactic, mito genic and angio genic with stimulatory effects, which help, in early closure of wound.

 
8.) Skin Grafting and BAT ( Bioengineered Alternative Tissue ) :
 

When superficial wound is healing but it is bigger in size then either skin grafts or BAT (e.g. Dermagraf and Apligraf) are used for early closure of wounds.

 
9.) Conventional and Specialty Dressings :
 

Our unit offers both conventional and specialty dressings like Medihoney, Allevyn, opsite, Tegaderm, Foam dressings and Acticoat Silver dressings etc.etc . We will assess your wound to determine the most appropriate wound dressing. As the wound heals, the dressing choice is adjusted. We have trained “Dressers” who can change dressing at home if our doctor advises. Later on you may be given instructions on how to change your dressing at home.

 
10.) Compression Therapy :
 

25% of the population has some form of venous abnormality. These patients are at risk for developing leg ulcers and often have swelling in their lower legs. Treatment of swelling is essential for wound healing to occur. There are several methods of applying therapy to reduce swelling, including compression therapy. Most appropriate compression therapy for wound is decided on individual basis.

 
11.) Off loading devices :

Off loading of wound is cornerstone for the management of non-healing diabetic foot ulcers. We at IWCC provide different types of Offloading shoes(Front, mid & back etc. )& Air Cast Walker, apply Total Contact Cast & advice other types of Offloading devices like Walker, Crutches etc. for speedy recovery of Foot Ulcers. Patients who require orthotics for pressure relief, support and prosthetic adjustments will be referred to an orthotics professional.

12.) Stem cells Therapy :
 

This is the most advanced ,sophisticated and latest modality for non-healing diabetic foot ulcers with peripheral vascular disease(PVD), Gangrenous or Pregangrenous Ulcers.

We are proud to inform that IWCC is one of the few centers in North India who has been performing this technique for recalcitrant ulcers.

In this technique bone marrow derived autologous stem cells are injected in calf Muscles which help in neo angiogenesis i.e. development of new blood vessels, leading to early wound healing.

By coordinating with other community services, our program offers additional support for our patients, including Blood sugar control, Diabetic education, nutritional education, physical therapy, occupational therapy, gait training and daily reinforcement and teaching related to each diagnosis.
